Understanding Sjögren's Syndrome From A Patient's Perspective

Sjogren's Syndrome is an autoimmune disorder in which immune cells attack and destroy the glands that produce tears and saliva. It can also adversely affect internal organs, and this damage is unseen as with some other autoimmune disorders. It is estimated to strike as many as 4 million people in the United States alone, making it the second most common autoimmune rheumatic disease.
The purpose of this blog is to provide a platform for those who have been diagnosed with or think they may have Sjogren's Syndrome, so that they may find help, information, and support from others. Autoimmune Diseases and Oral Disease
Autoimmune diseases are the result of the body responding in an inappropriate manner to normal tissues and/or substances present in the body, causing prolonged inflammation followed by tissue destruction. The body is fooled and can’t recognize the difference between healthy tissue and disease. The body, therefore, mounts an immune response (directs antibodies against its own tissues) against itself, as if allergic to itself. The cause of this dysfunctional behavior is unknown, it may be caused by a drug or toxin or bacterial or viral infection or environmental exposure to foreign substances. The result of the body’s inability to recognize the difference between normal tissue and disease results in destruction. As we age, this alone causes our immune system to decline in effectiveness. Autoimmune diseases affect over 24 million Americans and are one of our society’s leading causes of death and disability. Autoimmune diseases are ranked as the number one cause of heart disease, cancer, and all diseases.
A couple of the most common autoimmune diseases are diabetes (Type 1), rheumatoid arthritis, and allergies. Many autoimmune diseases also may have a genetic or traumatic component.

Autoimmune Disease Causes: The immune system is also thought to be suppressed by multiple factors, including abuse of:
- alcohol
- caffeine
- tobacco
- sugar (this cannot be over-emphasized)
- drugs
- food (poor diet or contaminated with herbicides, hormones, etc.)
- sleep (lack of)
As well as exposure to environmental pollution, including:
- automobile exhaust
- chemical fertilizers
- cigarette smoke
- heavy metal
- herbicides
- industrial waste
- pesticides
- stress
So, what has autoimmune diseases and oral disease (periodontal disease and tooth decay) have in common? Well, much more than one might think: Plaque and calculus left on teeth cause inflammation, initially gingivitis, followed often by periodontal disease (which we used to refer to as periodontitis). Any time you see –itis on the end of a word, it is screaming inflammation. Remember: it is always means it is inflammation. Chronic inflammation causes destruction of tissue. Periodontal disease causes destruction of all periodontal tissues, gum, bone, and periodontal ligament, causing loss of teeth. Tooth decay causes loss of tooth structure. Since periodontal tissues are all tissues supporting teeth in the mouth, without them – no teeth.
How periodontal disease (inflammation of all periodontal tissues) leads to loss at of oral tissues, including teeth, is a simple model demonstration for what happens to a body with autoimmune diseases.
Anything that a patient can do to minimize or eliminate inflammation will lessen the effects of the disease. A few of the things a patient can do to lessen the effects of many of these diseases are to avoid abuse of/or exposure to the above mentioned factors. Also, as always, eat lots of antioxidants found in fresh fruit and vegetables. Include vitamin C, vitamin E, green tea extract, beta-carotene, grape seed-skin extract, coenzyme Q-10 (coQ10) and selenium in your diet.
Also, talk to your dental and medical professionals on current treatments. Find support groups, ask others with your particular autoimmune disease how they cope.

Benefits of Organic Eggs
Organic eggs are extremely nutritious and contain significantly higher ratios of Vitamins A and E, Omega 3, and beta carotene than commercially produced eggs. They are also lower in cholesterol and saturated fat, making them an excellent choice for anyone concerned about their health. Benefits of Duck & Goose Eggs
Duck eggs are significantly larger than chicken eggs, and goose eggs are larger still. One goose egg, when cooked, will be roughly equivalent to four chicken eggs, and both duck and goose eggs also have more yolk than the average chicken egg. Thicker shells lend themselves toward a longer shelf life (at least 6 weeks in the refrigerator), and goose eggshells are so thick that they are perfect for the delicate art of carving and embossing beautiful designs directly onto the shell. Both duck eggs and goose eggs are slightly higher in protein content, as well as containing greater concentrations of other nutrients like iron, potassium, Vitamin E, Vitamin B12, and Vitamin D.
Duck and goose eggs have their own unique flavors as well. Sjögren’s syndrome is a common autoimmune disorder affecting nearly four million people in the U.S. If you have Sjögren’s syndrome, you may have dry mouth symptoms. Dry mouth (xerostomia) can have multiple sources, for example:

Sjögren’s disease is an autoimmune disease that attacks the exocrine glands of the human body. The most common first symptom of the disease is DRY MOUTH and dry eyes. Because of the gradual onset of the disease, most patients are affected by the disease for five to seven years before a diagnosis of Sjögren’s disease is made.
Your dentist is often the medical professional to first recognize the symptoms of this chronic disease. Your dentist is also a most significant professional to have to support you if you are diagnosed with Sjögren’s or any of the above listed maladies, which result in DRY MOUTH.
People with dry mouth should carry water with them where ever they go, have it readily available at work, in the car, and next to their bed. The simplest solutions to a problem often offer the most immediate benefit.
Dry mouth leaves teeth without the protective enzymes found in saliva, making teeth much more prone to rampant decay. We, as dentists, can fluoridate teeth in various ways, to remineralize teeth and make them more resistant to tooth decay.

Toothbrushes and Toothpaste
Toothbrushes:
Frequently, I am asked what is the best toothbrush and toothpaste. As with many things in life, the answer is not so simple. But there are a few guidelines.
Definitely, I would never recommend any hard-bristled or medium-bristled toothbrush, only soft-bristled toothbrushes for human teeth. Hamster cages and boots can be cleaned with hard or medium bristled toothbrushes, but not teeth. Also, the bristles should be rounded on the end, not sharp or ragged. Gums can be lacerated easily with a sharp bristled toothbrush.
SOFT: Soft nylon will prevent teeth and gums from being scratched. Tooth- brushes with the American Dental Association’s Seal of Acceptance (ADA Seal) should be your only reasonable choice for toothbrushes (and tooth- paste) for that matter.
RIGHT SIZE: The size and shape of the brush should also be user appropriate. Children need smaller brush heads and handles. Your brush should feel comfortable in your hand. Smaller brush heads in adults is usually better. Ask your dentist or hygienist if you have a question about your brush or brushing.
MAINTAIN: Replace your worn or frayed brush at least every 3-4 months. Frayed bristles can damage teeth and gums and harbor bacteria.
ELECTRIC: And, if you need (children and handicapped patients) electric toothbrushes work as well as manual brushes. So…if you’ve followed the above guidelines, the best brush is the one you like to use at least twice daily.
The average person brushes for about 37 seconds. However, to do a proper job it takes two to three minutes to sufficiently remove plaque when brushing. A three minute egg timer is a good reminder for children’s brushing. Aim the bristles at a 45o angle to the long axis of the teeth and with gentle circular motions, brush all exposed surfaces.
DEVELOP A BRUSHING PATTERN: Brush in a pattern that covers all the surfaces of all teeth each time you brush. For example, brush from right to left on the outside of upper teeth, then left to right inside surface of upper teeth, then outside right to left lower teeth and then inside left to right lower teeth and then lower biting surfaces left to right and upper biting surface right to left.
TOOTHPASTE: Tooth paste not only polishes teeth, it also helps remove plaque (bacteria and its waste products) from teeth. Daily removal of plaque from teeth helps keep teeth and gums healthy and breath fresh.
AMERICAN MADE: This is one place where the American Dental Association (ADA) seal is particularly important. Never use toothpaste from China.
WHITENING TOOTHPASTES: Teeth whitening toothpastes don’t really work to whiten teeth. They may rid you mouth of stains. However, they often cause sensitivity.
DESENSITIZING TOOTH PASTES: These toothpastes are valuable in blocking irritants from getting to nerve ending. Potassium nitrate (salt peter) is the most effective desensitizing agent in desensitizing toothpastes (potassium nitrate is also used to cure and maintain red color in corned beef).
FLUORIDE TOOTHPASTE: Fluoride is the most significant chemical element which can be easily added to tooth paste to improve dental health. Fluoride remineralizes and strengthens teeth as well as desensitized teeth. Children, especially through teen years, and senior citizens can benefit from fluoride in toothpastes and other forms of topical fluoride, especially in areas (there are still a few) without the benefit of community water fluoridation.

Making Christmas Merry
Christmas 2011 provides a powerful opportunity for everyone wishing to live the true meaning of Christmas. Americans are some of the most generous people on earth. Citizens of Spokane, Washington have proven time and again that they are exceptionally generous. The economy this year has had a dramatic effect on the tone of the holiday in many homes. People who have always celebrated in abundance are now cutting back or going without. Not just are members of the middle class learning to tighten their belts, but the numbers of those without homes is continuing to swell. Statistics of those in need are reported regularly. Such statistics create an opportunity for the more blessed majority to make Christmas merry for themselves and the multitude less fortunate. There are plenty of people who truly need help, this year even more than last. Each member of any family who feels blessed this Christmas should make a conscious commitment to give part of what may have been given to each other, to someone or a family in greater need.
Often gifts are given because the exchange is expected. What if as a group your family did something for a needy person or other needy family? You will truly be amazed how much more fulfilling your Christmas will be. The giver and the gifted both will find Christmas more fulfilling.
The joy of Christmas season is best exemplified when Christ is kept in Christmas. I once had someone tell me how to keep joy in Christmas. I was told to think of joy as shorthand for the priorities of Christmas: Jesus first, Others second, and You third. JOY. If we all remember that order I’m sure more Christmas joy will be had by all and we will make Christmas 2011 merry.

Life Insurance Tips
Sjögren’s Syndrome is not an easy thing to live with all of the time. Symptoms can range from mild to extreme, but in any case it can complicate life. Those who are afflicted with extreme cases know the pain and suffering they have to go through. That suffering includes dry mouth, rashes, pancreatic shut down, digestive issues, effects on major organs, and other auto-immune diseases such as rheumatoid arthritis. These extreme symptoms can cause debilitating outcomes and early death. For many with severe symptoms they consider the outcome and want to make sure they can provide for their family no matter what happens to them. Now things such as life insurance become more important.
For those in the United States, trying to get life insurance can be quite difficult for someone who has a disability or disease. Whether it’s Sjögren’s Syndrome, Diabetes, or another issue, it complicates the application process and often raises insurance premiums so high that disabled people can’t afford to pay for them. It’s all related to the amount of risk and profit-and-loss statements of the insurance provider.
Here are some tips to help anyone applying for life insurance with a disability or disease.
Talk to an insurance professional that specializes in “impaired risk”. If you can’t locate one, ask your current insurance agent or others that you trust.
Ask if the insurance agent checks with multiple companies to give you the best rates. Rates are determined by which category of risk you are put in.
Submit a cover letter with your application. It is important to get the best rate that you get put into the best category that you can. By reading a piece of paper a life insurance underwriter isn’t necessarily going to get the best picture. Maybe your disease hasn’t progressed as far as someone else’s or maybe you take a more proactive approach with your treatment or healing. That could extend your life longer and make your file not as risky as another person’s file. The underwriter can’t tell from your answers to a few basic questions.
In your cover letter, lay out exactly how far your disease has or has not progressed, how you do things differently, and what you are planning to do in the future to keep up the positives in your treatment. Tell them any other details that might show them you are less risky. However, by any means, do not fabricate lies fit a category that does not apply.
Holidays and Dental Health
WOW! The Christmas holiday is rapidly approaching, which catches people off guard on how quickly 2011 ends and 2012 is upon us.
In the hustle-bustle of the hectic holiday season, many may forget about oral health maintenance. Dental insurance usually is contracted yearly, from January 1st through December 31st. Therefore, if you have dental insurance coverage that you or your employer has paid for, it needs to be taken advantage of before December 31st. Use it or lose it.
Many people think, “I’ll do the dental treatment I need in January, after things settle down.” Well … if you see your dentist and have dental treatment done before year’s end, you could have the insurance benefit of 2 years (one before Dec 31st and one after Dec 31st). Either way, you or your employer have paid for your dental insurance (if you’re lucky enough to have dental insurance), so you surely want to take full advantage of your benefits.
Whether or not you have dental insurance, maintenance is always cheaper than repair. As the conclusion of 2011 approaches, be diligent in assuring that you’ve followed the old adage: Brush your teeth twice a day, and see your dentist twice a year.
Dental health is more than just a dazzling smile. Good oral health can help minimize the effect of diabetes, heart disease, respiratory diseases, osteoporosis, and Alzheimer’s disease.
Enjoy the holiday season. Be aware of all the things that you can do to improve your overall health. We are all exposed to excessive amounts of good and not so good foods during this festive season. Be careful how you fuel your systems and understand that oral health is a most significant part of maintaining overall health.
On a final note, Christmas is a season to reflect on how many blessings we have. But in the interest of keeping Christ in Christmas, it is a time to generously be Christian. With our present economy, more people than ever are in need. Generously help someone in need. Perform random acts of kindness. Go out of your way to put smiles on faces of people you may not even know.

HAPPY THANKSGIVING!
Again, in 2011, the Thanksgiving holiday has come upon us quickly. The Christmas tree lot lights are set up and snow and ice greet us on our morning commutes.
A truly American holiday, Thanksgiving is a national day for us to express our gratitude for all the freedoms with which we have been gifted. This is a time to reflect on the many liberties we often take for granted.
Thank You to our service men and women who make our republic safe. Thank You to our forefathers with the vision to create a Constitution protecting our freedoms. Every person has the right to do what they wish with their life. Work hard and get ahead. Thank You for the Declaration of Independence which says all men are created equal, with the right to life, liberty, and the pursuit of happiness. Thank You.
Our country is a religious country with freedom for everyone to practice whatever religion they wish. And appreciative Americans pray often the shortest prayer on earth: Thank You. But, Thanksgiving is not a religious holiday. Americans are naturally thankful. Religious or not, Americans realize how fortunate we are to live in such a beautiful country, rich in natural resources, with religious and civil freedoms for all. Thank You is a tremendously American expression.
And if there be anyone who wishes to know how to have a Happy Thanksgiving, merely analyze the expression. It is by giving thanks that we make ourselves and others happy. There are plenty of people that have less than “we,” no matter who “we” are. Therefore, to have a truly Happy Thanksgiving, simply give thanks by doing something for someone who has less than “we” do. “Amen, I say to you, whatever you did for the least brothers of mine you did for me.” Find someone who needs anything and be the provider of that need. Such giving can be anonymous, which can be most rewarding. Give thanks often.

Mountain Valley View Farm is proud to be a small, family owned and operated farm dedicated to raising beautiful Icelandic horses and growing fresh, organic produce that we sell at local farmers’ markets. We also have a small herd of Nubian dairy goats and Babydoll Southdown sheep that provide us with rich, creamy milk from which we craft a line of delicious artisan cheeses and handmade soaps that nourish and rejuvenate the skin.
Our Getaway Studio features a beautiful Bed & Breakfast where you can relax in a peaceful country setting, as well as the Dining Room that is open year-round for catering, hosting small dinner parties, and cooking classes taught by Karen Jean Matsko Hood, author of the popular Cookbook Delights series.
In our fully-licensed commercial kitchen, we use time-honored family recipes to make small batches of scrumptious artisan baked goods, candies, jams, jellies, fruit butters, and syrups from high quality local and organic ingredients that have a fresh, unbeatable taste. We also craft lovely gift items, including cookie and soup mixes packaged attractively in jars, sachets of lavender and potpourri, and our signature potpourri strings that add a decorative touch to any room.
As you can tell, there is always something new and exciting happening at Mountain Valley View Farm! Of course, we also have a great selection of organic produce and fresh eggs from the free-range chickens, ducks, geese, and turkeys that live right here on our farm.
